California State Required Continuing Education for Physician Relicensure

Physicians practicing in California must complete continuing medical education (CME) as required by the Medical Board of California (MBC) to maintain an active medical license. California's CME requirements are among the more comprehensive in the nation and reflect the state's commitment to ensuring physician competency and patient safety.

CME Requirements for California Physicians

Overall CME Hours

Physicians renewing their medical license in California must complete a minimum of 250 hours of CME credits over a three-year renewal period (approximately 83-84 hours per year). These credits must come from approved providers and accrediting organizations recognized by the Medical Board of California.

Required Content Areas

Of the 250 required CME hours, California mandates that physicians complete specific coursework in the following areas:

  • 12 hours in Physician-Patient Communication and Medical Ethics
  • 12 hours in End-of-Life Care
  • 8 hours in Risk Management and Patient Safety
  • 4 hours in Cultural and Linguistic Competency (completed within current renewal period)
  • 4 hours in Unconscious Bias and Culturally Sensitive Care (for certain licensees)
  • 3 hours in Prescribing Controlled Substances Safely (including opioid management and addiction)
  • 207 additional hours in accredited medical education relevant to the physician's practice

Board-Approved Credit Sources

California accepts CME from the following accredited sources:

  • AMA PRA Category 1 Credits™
  • AAFP Prescribed Credits or Elective Credits
  • American Osteopathic Association Credits
  • Medical Board of California-approved CME providers
  • University-sponsored continuing medical education programs
  • Hospital medical education programs
  • State and specialty medical society programs
  • Board certification maintenance programs (MOC credits)
  • Teaching activities and clinical instruction (with limitations)

Renewal Cycle and Timeline

California uses a three-year renewal cycle, with renewal dates based on the physician's birth month. Physicians must:

  • Complete all 250 CME hours within the three-year renewal period
  • Maintain current malpractice insurance coverage
  • Submit renewal applications through the Medical Board of California's online system
  • Pay the required renewal fee
  • Verify current practice information and address

Important: All CME hours must be completed and documented before your license expiration date. The Medical Board of California does not grant automatic extensions for incomplete CME requirements.

Specific Course Requirements - Important Details

Cultural and Linguistic Competency Requirement

California requires 4 hours of CME in cultural and linguistic competency to be completed during the current renewal period. This requirement emphasizes the importance of culturally sensitive care and effective communication with diverse patient populations.

End-of-Life Care Requirement

The 12-hour end-of-life care requirement covers topics such as advance directives, palliative care, pain management at end of life, and physician-assisted death where applicable under California law.

Prescribing Controlled Substances Safely

All physicians who prescribe controlled substances must complete at least 3 hours of CME focused on safe prescribing practices, including information on opioid addiction, alternatives to opioids, and proper pain management strategies.

Documentation and Record-Keeping

California physicians are required to maintain comprehensive documentation of all CME activities, including:

  • Certificates of completion or official transcripts
  • Course title, date, and provider information
  • Number of credit hours earned
  • Accreditation designation (AMA PRA Category 1, etc.)
  • For online courses: confirmation of course completion

The Medical Board of California may request verification of CME hours at any time during the renewal process or during a compliance audit. Physicians should retain all documentation for at least six years from the date of course completion.

Special Circumstances

Military Service

Physicians on active military duty serving outside California may apply for a waiver or deferral of CME requirements. Appropriate military documentation and timing requirements apply.

Newly Licensed Physicians

Physicians initially licensed during a renewal cycle will have their CME requirements prorated based on the time remaining in the renewal period.

Maternity or Disability Leave

Physicians taking documented medical leave may be eligible for a waiver or extension. Requests must include supporting medical documentation and be submitted to the Medical Board of California.

Retired Status

Physicians who retire from active medical practice may apply for retired status, which exempts them from CME requirements. However, reinstatement to active status requires completion of CME requirements.

Consequences of Non-Compliance

Failure to meet California's CME requirements may result in:

  • License renewal denial or delay
  • Administrative fines and penalties
  • License suspension or revocation
  • Disciplinary action from the Medical Board of California
  • Inability to practice medicine in California
  • Potential loss of hospital privileges and insurance coverage

Physicians should actively track their CME hours throughout the renewal period and plan accordingly to ensure full compliance.

Online vs. In-Person CME

California accepts both online and in-person CME courses from approved providers. However, physicians should verify that:

  • The provider is Medical Board of California-approved
  • The course meets content requirements (especially for mandated topics)
  • Online courses provide verifiable completion documentation
  • Self-assessment activities are appropriately credited
